Every activity sponsored by Public Church is carefully planned and supervised by responsible adults. However, despite diligent planning and precautionary measures, unforeseen events may occur.

By signing this form, I, the parent/guardian, acknowledge and accept all risks and hazards associated with participation in church-related activities. I agree to release, indemnify, and hold harmless Public Church, its employees, volunteers, and representatives from any claims, damages, losses, or injuries to the person or property of my child arising from participation in these activities, except in cases of gross negligence or willful misconduct.

Furthermore, I understand that this agreement serves as both a medical consent and a liability release for the minor named above.

In the event that I cannot be reached during an emergency involving my child, while attending Journey Camp, I hereby authorize the church leadership to act on my behalf to secure appropriate medical or dental care. This includes, but is not limited to, the selection of a licensed physician, dentist, or healthcare provider to hospitalize, provide necessary treatment, administer medications, or perform any procedures, including anesthesia or surgery, as deemed medically necessary for the health and well-being of my child.

I understand that every reasonable effort will be made to contact me prior to such action. However, in situations where time is critical or I cannot be reached, I grant this permission to ensure my child's care and safety.
